• Sub-Acute Step-Down from Rehabilitation/LTACH: Individuals who have completed maximum physical therapy benefits but still require 24/7 personal care and medication administration.

Under our assisted-living licensure and individualized care plans, our Step-Down Care Program supports a wide range of specialized medical needs:

• Advanced Device & Airway Support: Tracheostomy maintenance, stable ventilator oversight, continuous oxygen administration, and suctioning.

• Enteral & Intravenous Management: PEG/G-tube care, enteral formula feeding, IV therapy, and PICC/central line site maintenance.

• Specialized Wound Care: Negative Pressure Wound Therapy (Wound VAC) and complex post-surgical dressing changes.

• Medication & Clinical Supervision: Sliding-scale diabetes monitoring, insulin administration, drain management, and LVAD monitoring support.

• Pre-Arrival Equipment Delivery: We coordinate with medical suppliers and pharmacy partners to ensure all specialized equipment, tubes, and medications are on-site prior to patient arrival.
